Living with dementia can change a person’s memory, communication and day-to-day needs. We provide care at home that follows the person’s preferences, strengths and routines, with support for family carers where appropriate.

Familiar routines and individual support

We aim for consistent carers and visit times, while planning for illness, leave and changing needs. We get to know how the person prefers to communicate and what helps them feel comfortable. A care plan is agreed and reviewed with the person and those involved in their care, as appropriate.
